When the exacerbation of acute left ventricular failure develops, the neuroendocrine responses including sympathetic nervous system and the renin-angiotensin system leading to fluid volume excess are activated to maintain perfusion of the vital organs (Lemone & Burke, 2013). The decreased contractility of the left ventricle causes decreased left atrial pressure and moreover, pressures in pulmonary veins increase. Pulmonary capillary hydrostatic pressures rise, ultimately exceeding the osmotic pressure of the blood (Eugenio, Luna & Mihai, 2010). In other words, left ventricular failure leads to reduced cardiac output (CO) causing reduced effective renal perfusion. Decreased renal perfusion causes the activation of renin-angiotensin system and leads to vasoconstriction and stimulates the adrenal cortex to produce the secretion of aldosterone and the posterior pituitary to produce ADH. Aldosterone encourages sodium resorptions in renal tubules, prompting water retention (Craft, Gordon & Tiziani, 2011).
