1. 장내균총이란?
어머니에게서 태어난 신생아는 태어난 후 몸밖의 피부를 비롯해 구강, 장관, 상기도, 질 등 바깥 세계와 통하는 과정에서 여러 가지 생리적 인자와 외부 환경의 영향에 의해 성장한다. 사람이 유아에서 성인이 되는 과정에서 생리적 인자와 외부 환경의 인자에 의해 미생물은 나타난다. 그리고 생식 장소의 조건에 적응한 균군만이 살아남아 살게 된다. 이를 상주균군(normal flora)라 하고, 사람 고유의 미생물군으로서 평생 사람과 공생하고 존재한다.
신생아가 태어난 직후의 태변은 거의 무균적이지만 탄생 다음날에는 용변중에 10^10/g 이상의 미생물을 볼 수 있다. 불과 24시간 이내에 장관에 침입하는 미생물은 대장균을 시작으로 장내 세균, 포도상구균 등이며, 생후 3~4일 째에는 비피더스균이 출현하고, 곧 장내 세균은 점차 감소해, 신생아 장관내에는 빞피더스균을 우점종으로 한 정상 미생물군이 정착한다.
장내균총의 대표적인 균종들은 Bifidobact-eria, Bacteroides, Enterobacteria, Lact-obacilli, Streptococci 등이고 20대를 기준으로 유아에서 노년에 이르기까지 Bifidob-acteria에서 Bacteroides로 우세균이 바뀐다. Figure 1-1을 보면 출생에서부터 노년기까지의 다양한 균의 종류의 수가 어떻게 변화하는지 알 수 있다.
장내는 일정량의 영양분이 구강을 통해서 계속 공급되고 항문을 통해서 일정량의 균수가 배출되는 연속배양장치이며 이 연속 배양장치에는 이미 증식한 균주들이 포화되어 있다고 생각하면 좋겠다.
소화관에 서식하는 장내세균총들을 보면 장내세균은 일단 정착하면 크게 변동하지 않는 정주균(상주균)이 있고, 2~3일 내에 자취를 감추는 통과균이 혼재되어 서식하고 있다. 정주균은 숙주와 특수한 관계를 갖고있는 것으로 고유균이라고 할 수 있다. 사람의 장내에 정착하고 있는 우세균인 Bacteroides,Bifidobacterium, Eubacterium, anaerobic Streptococcus 등등. 통과균은 장내에 계속해서 사는 것이 불가능하고 배설된다.
